Alternate Income Form FAQ

  • The Alternate Income Form is a household form for families enrolled at all SUSD Schools.
    This form will determine how much education funding our district will receive.

    • SCHOOL FUNDING. We need all families to fill out this form so that all SUSD Schools receives the necessary resources to continue to provide all students with the highest quality education, including funding for things like computers, books, school supplies, and more.
    • TECHNOLOGY FUNDING. all SUSD Schools receives more funding for internet access and computers.
    • GROCERY BENEFITS. Filling out this form may help provide your family with money for groceries through P-EBT.
    • SAT, ACT, AND AP TEST DISCOUNTS. If your household qualifies, you can receive discounts for academic testing.
    • COLLEGE APPLICATION DISCOUNTS. If your household qualifies, you can receive discounts for college application fees.
    • ATHLETICS. If your household qualifies, you may receive discounts for fees associated with participating in all SUSD School athletics.
  • No, only one (1) form per household needs to be completed. Please include the names of all students enrolled at all SUSD Schools on the form.

  • Yes, the personal information you provide on the Alternate Income Form is kept strictly confidential.

  • Please complete the form by September 26, 2025.

  • No. Submitting the Alternative Income Form does NOT change or impact your child’s access to free meals in any way. Meals will remain free for all students at SUSD (CEP) schools, whether or not the form is completed.

  • CEP (Community Eligibility Provision) allows schools with a high percentage of low-income students to offer free meals to all students—no application needed. In the traditional meal program, families had to submit a Free/Reduced-Price Meals application to qualify. CEP simplifies this—but we still need income information (via the Alternative Income Form) for funding purposes.
    So while CEP covers meals, the Alternative Income Form supports funding for education.

  • Yes! We encourage all families to fill out the form, even if you think you may not qualify. Every completed form counts and helps ensure accurate data and maximum funding.
