Looking for a public record? Several departments across Sunnyside Unified School District process different types of records requests. Please review the sections below carefully and select the option that best matches the records you need. Submitting your request to the appropriate department will help ensure it is routed correctly and processed as efficiently as possible.

 

Public Records Request

The Sunnyside Unified School District Governing Board recognizes the public’s right to information concerning district actions, policies and the details of educational and business operations. Public records of the district will be open for inspection by any person in accordance with Arizona Public Records Law.  

In accordance with ARS 39-121 through 39-121.03, community members may submit Public Records Requests as outlined under Governing Board Policy KDB: Public's Right To Know / Freedom Of Information.

Student Records

If you are needing a copy of your student records, please contact Debbie Burr in student records at debbieb1@susd12.org
Please include a copy of a picture ID and details on what records you are needing along with your contact information.


 

Special Education Records Request

For currently enrolled students, or out of the district for less than five years, contact your child's school or the SPED office (PHONE #)
If you are looking for older SPED records (student out of district for more than five years) contact Debbie Burr in student records at debbieb1@susd12.org


 

Employment Verification, Personnel Records, and other matters

The following personnel request shcoul go through the Human Resources Department.
Please contact Michelle Mendivil at michellem1@susd12.org for any of the following:

  • Address/Phone Number Changes.
  • Review of personnel files.
  • Copying of documents contained in personnel file (upon written request).
  • Teacher certification record updates.
  • Submission of Professional Development forms required for recertification.

While Human Resources notifies teachers of their certification renewal needs, it remains each certificated individual's personal responsibility to remain abreast of their own certification renewal requirements and to ensure their own compliance to avoid loss of certification. Questions regarding certification or re-certification can be directed to the Human Resources Department, as well as the State Department of Education Certification Unit.
